Take Action for Arizona's Children through Care Coordination: A Bridge to Action Take Action for Arizonas Children through Care Coordination: A Bridge to Action Background: Children with Special Health Care Needs (CSHCN) have chronic physical, developmental, behavior or emotional conditions and they require health and related services more than are required of children without these conditions. This project is designed to address the families needs for care coordination through creating a state-wide system that can be tested through PCOR. Proposed Solution to the Problem: We will build the capacity of our existing CSHCN coalition to conduct patient-centered outcomes research (PCOR) during five regional conferences. Our Award will allow our coalition to create an infrastructure to address the needs for care coordination as well as expand the coalitions ability to conduct PCOR on care coordination. Our conferences will be in Flagstaff, Phoenix, Tucson, and Yuma, all cities in AZ that represent very diverse populations within AZ. Our settings will be hospital and clinic meeting rooms, university classrooms, and city venues such as libraries.
