Arizona State University, Southwest Interdisciplinary Research Center hereby agrees to work in partnership with the Urban Indian Coalition of Arizona, Phoenix Indian Center and their partners (Tucson Indian Center and Native Americans for Community Action) on activities specifically related to the Parent Training Project as funded by the Arizona Parents Commission on Drug Education and Prevention Grant Program.

SCOPE OF WORK For ASU, Southwest Interdisciplinary Research Center (SIRC) For the Period July 1, 2013 to June 30, 2014 (Continuation Year 4 of 6, under new agreement for years 4-6) To perform Training and Evaluation Services For the Urban Indian Coalition of Arizona (UICAZ), Phoenix Indian Center and their partners. Arizona State University, Southwest Interdisciplinary Research Center hereby agrees to work in partnership with the Urban Indian Coalition of Arizona, Phoenix Indian Center and their partner, (Tucson Indian Center and Native Americans for Community Action) on activities specifically related to the Parent Training Project as funded by the Arizona Parents Commission on Drug Education and Prevention Grant Program. They are responsible for providing the following: a. Training and evaluation services to include training of facilitators and observers on (1) adapted FPNG curriculum and evaluation, Health curriculum, (2) Outreach& Recruitment b. Coordination of activities with the staff at lead agency, the Phoenix Indian Center, Inc. to include staff activities, weekly meetings with all UICAZ and travel to site for site visits and administration of surveys c. Advisement on the implementation plan by the Urban Indian Coalition of Arizona d. Development of survey instruments; revised instruments for the adapted curriculum e. Entry of data and analysis f. Evaluation reports to disseminate findings
