SCOPE OF WORK For ASU, Southwest Interdisciplinary Research Center (SIRC) For the Period January 2012 to August 31, 2012 (Continuation Year 2) And for the Period September 1, 2012 to August 31, 2013 (Continuation Year 3) To perform Training and Evaluation Services For Maricopa County Department of Public Health, Streets of Success Program January 2012 to August 31, 2012 Arizona State University, Southwest Interdisciplinary Research Center hereby agrees to work in partnership with the Maricopa County Department of Public Health on activities specifically related to the Streets of Success Program (MC-SOS) as funded by the Office of Adolescent Health. The tasks and deliverables described in this scope of work will be incorporated into the independent external evaluation that will be conducted by the Southwest Interdisciplinary Research Center (SIRC) at Arizona State University. This local evaluation will be under the leadership of Dr. Wendy Wolfersteig, Evaluation Project Director. The goal of this study reflects the dual purposes of measuring (1) replication with fidelity of TOP and (2) the overall effectiveness of the MC-SOS program.
