Team-Centered Care for Vulnerable Populations: a comprehensive approach to educating and supporting primary care teams Team-centered Care for Vulnerable Populations: A comprehensive approach to educating and supporting primary care teams Our objectives for this grant are to: 1. Develop and beta test two components of a new prototype for a comprehensive team-centered education intervention for primary care teams serving vulnerable and underserved populations. 2. Create an effective and efficient model for academic-clinical co-creation/collaboration on competency and evidence-based interventions ready for clinical application. 3. Establish the foundation for seeking funds to expand and test the intervention across primary care clinical settings.
