Blue Prints 2.0 Blue Prints 2.0 1.Revise, as necessary, the one year evaluation plan submitted by to Substance Abuse and Mental Health Services Administration/Center for Substance Abuse Treatment (SAMHSA/CSAT). The evaluation plan will include process and outcome indicators, as well as GPRA reporting requirements. Completion Date: Within 15 days of executed contract. 2. Submit necessary human subjects protection approval forms and materials the Arizona State Universitys Human Subjects Committee for review and approval. Completion Date: Within 15 days of executed contract 3. Provide quarterly trainings and technical assistance on data collection and the administration of survey instruments and evaluation protocol. Completion date: Quarterly and upon request. 4. Conduct evaluation activities including key informant interviews, focus groups, participant/site observations, report findings quarterly at meetings with PI, and review of key documents. Completion Date: TBD 5. Provide quarterly evaluation activity reports to the Principle Investigator by the 15th of the month following the end of the quarter. Completion Date: January 15, 2011; April 15, 2011; July 15, 2011 and October, 2012 (final report).
